Declaration of Consent & Participation*
1. Voluntary participation
I, as the parent or legal guardian of the participant named above, confirm that I am authorised to register my child for the SUSA AUTUMN PROGRAM 2026.
I understand that football is a physically active sport involving running, sprinting, jumping, changes of direction, ball contact and physical interaction with other participants.
Football is generally considered a reasonably safe sporting activity when appropriate safety rules, coaching instructions and sporting conduct are followed. Nevertheless, participation involves inherent risks that cannot be completely eliminated.
These risks may include, among other things:
falls and collisions;
muscle, ligament, tendon or joint injuries;
bruises, cuts or abrasions;
sprains and strains;
injuries resulting from contact with another participant;
injuries resulting from the playing surface, equipment or weather conditions;
fatigue, dehydration or heat-related effects;
and, in rare cases, more serious injury.
I understand and accept that the possibility of injury exists even where reasonable safety measures are taken.
2. Fitness and ability to participate
I confirm that, to the best of my knowledge, my child is physically capable of participating in the activities for which they are registered.
I will inform SUSA before participation if my child has a physical limitation, injury, medical condition, medication requirement or other circumstance that may materially affect safe participation.
I understand that SUSA may, where reasonably necessary for the safety of my child or other participants, modify, restrict or discontinue my child's participation in an activity.
3. Responsibility of the participant
I agree that my child will:
follow reasonable instructions given by coaches and SUSA staff;
behave respectfully towards coaches, staff and other participants;
use the facilities and equipment appropriately;
follow applicable safety and sporting rules;
inform a coach or member of staff if they feel unwell, injured or unable to continue safely.
SUSA reserves the right to exclude a participant from an activity where their behaviour presents a serious or continuing safety risk to themselves or others.
4. Medical and emergency assistance
Every participant has to have a valid insurance in Austria. In the event of an accident, injury or medical emergency, SUSA will take reasonable steps to provide or arrange appropriate first aid and, where necessary, contact the parent/legal guardian and/or emergency services.
I authorise SUSA and its responsible staff to take reasonable emergency measures where immediate action is necessary and I cannot be contacted in time.
This provision does not constitute a guarantee of medical treatment or a waiver of any statutory rights.
5. Responsibility and liability
I acknowledge the ordinary and inherent risks associated with participation in football and agree to take reasonable precautions to reduce those risks.
Nothing in this declaration excludes or limits any liability that cannot lawfully be excluded or limited under Austrian law.
In particular, this declaration does not exclude liability for:
intentional conduct;
legally non-excludable liability;
personal injury where exclusion or limitation is prohibited by applicable law;
or other cases in which Austrian mandatory law prevents a limitation of liability.
SUSA and the organisers remain responsible for fulfilling their legal duties of care and for taking reasonable measures appropriate to the organisation of the program.
6. Personal belongings
Participants should not bring unnecessary valuables to training or program activities.
SUSA is not responsible for the loss of or damage to personal belongings except where liability exists under applicable law.

QUESTION TWENTY three
Photography & Media Consent*
SUSA also endeavors to capture the action, energy and spirit of our programmes through controlled photography and video recording.
Photographs taken during SUSA training sessions, camps, matches, events and other activities may be used to document and promote the work of SUSA. Selected photographs may appear on SUSA's: Website, Social Media, Flyers and printed promotional materials
Other official SUSA communication and promotional channels
In addition, SUSA may provide a selection of photographs taken during activities to participating families for personal memories and non-commercial use. SUSA will make reasonable efforts to ensure that photography is conducted appropriately and respectfully and that children are not portrayed in a manner that is inappropriate, degrading or harmful.
OPTIONAL CONSENT
Photography consent is voluntary and is not a condition for participating in SUSA programmes.
I consent to photographs and/or video recordings in which my child may be identifiable being taken and used by SUSA for the purposes described above, including SUSA's website, social media, printed promotional materials and communications.
I do not consent to my child being identifiable in photographs or video recordings used for the promotional purposes described above. I understand that I may withdraw my consent at any time for future processing by contacting SUSA. Withdrawal of consent does not affect the lawfulness of processing carried out before withdrawal.

IMPORTANT — NO FINAL MEMBERSHIP CONTRACT
Submission of this online pre-registration form does not by itself create a final or binding membership, training or participation contract for the full season/program. The player will initially participate on a two-week try-out basis.
A final contractual relationship for continued participation shall only arise once SUSA Vienna and the family have both confirmed the player’s continued participation, terms and conditions have been signed by the parents / guardians and the applicable fee has been paid.
Until these conditions have been fulfilled, the pre-registration should be understood as an application/request to participate in the try-out process and not as a commitment to a full season or programme.
